This is a case report about a 86-years old woman on chronic therapy with dabigatran 110 mg BID for a permanent atrial fibrillation, that, after an accidental fall, presented a falx subdural haematoma. In this patient the plasma level of dabigatran was measured 72 hours from last intake and it resulted still high: that’s why the specific reversal treatment with idarucizumab was administrated, despite the delay. The aim is to set up a hospital-wide policy concerning bleeding management under NOAC, so that the right strategy can be implemented as soon as possible.
